Summary:
This innovation delivers comprehensive medication management through AltaMed Health Services for the highest risk patients in the health system. These high risk patients are individuals who were recently or are frequently hospitalized, and patients who met other criteria based on a series of algorithms developed by the USC School of Pharmacy and embedded into AltaMed’s electronic health record system. The process allows for the identification of potential gaps in medication-related quality and/or safety such as drug- interactions, contraindications, or polypharmacy and result in an automatic enrollment into the comprehensive medication management program.
