By participating in the Let’s Get Healthy California’s “Share Your Story” you agree to abide by the following terms and conditions:
Each Participant hereby irrevocably and perpetually grants to the California Health and Human Services Agency (CHHS) and its Departments, the right and permission to take, use and publish any “Share Your Story” submission materials, including Participant’s name, organization, photographs, story title and story summary (in whole, or in part) for the Let’s Get Healthy California initiative or other governmental purposes. These purposes may include, but are not limited to, training, outreach, education, and media purposes.
Participant grants permission to CHHS and its Departments to modify and/or crop the photographs, and to paraphrase, shorten or amplify, and/or put his/her “Share Your Story” submission materials into conversational form. Participant gives permission to use any of his/her submission materials in combination with or as a composite of other matter, including, but not limited to, text, data, images, photographs and photographic reproductions, illustrations, animation and graphics, video or audio segments, and to make derivative works to be used in publications, broadcasts, public service messages and advertisements, press kits, letters, presentations, websites, social media, or through any form of media that CHHS and its Departments chooses to further governmental purposes.
Participant hereby waives any right that he/she may have to inspect or approve the finished product or products and the advertising copy or other matter that may be used in connection with them or the use to which they may be applied.
Participant agrees that the State will own any and all rights in any materials developed using the “Submit a Story” submission materials. Participant acknowledges that no monetary compensation is or will be payable to him/her for the use of his/her story.
Participant releases the State of California, its employees, contractors, agents, successors, and assigns from any and all liabilities, claims and demands arising out of or in connection with their use of the “Share Your Story” submission materials including but not limited to, claims for invasion of privacy, defamation, and infringement of the right of publicity.
Participant understands that his/her participation in Let’s Get Healthy California Website’s “Share Your Story” is strictly voluntary and that his/her eligibility to participate in any State program will not be affected by his/her choice in participating in Let’s Get Healthy California’s “Share Your Story.”
