Summary:
The Health Home Community Model Innovation is a multi-faceted model created to address needs of the homeless population. It serves the eligible homeless population through a comprehensive living support arrangement. Services include comprehensive care management, care coordination, health promotion, comprehensive transitional care/follow up, patient and family support, and referrals to community and social support services. The innovation also focuses on providing care to those with two chronic conditions, or one condition and at-risk of another, or a serious/persistent mental health condition.
