Summary:
This program is designed to reach out to minority populations to educate them on end of life options, including palliative care. The proposal involves training individuals on end of life care options and then targeting specific racial and cultural groups that are under-represented in hospice care populations. The goal is to increase awareness and placement of these populations in health care settings.
